ClaimZone Reporter can be used to report workers compensation claims for all 50 states. It can also be used to collect First Reports for other areas, including Guam, Puerto Rico, Longshore form, and others. Take a look at the accompanying screen shot,

and note the thumbnail picture of the state form in the left column. When you pick the state you wish to report, ClaimZone Reporter will generate the list of questions for that state exactly as they appear on the paper version of the form. You can see the paper version of the form by clicking the thumbnail. This is very helpful for people filling out claims.
